ABSTRACT

In recent years, the field of Narrative Pharmacy was introduced, which particularly addresses the pharmacist not only to guide a relationship of listening to and caring for the patient but also to strengthen and motivate toward the profession, improve relationships with colleagues, enhance the ability to teamwork, and understand emotions. In this paper, we report the analysis behind the construction of the Value Chart from the personal narratives of members of the Italian Society of Hospital Pharmacy. Each member's subjective professional experiences and their own view of themselves within society were collected through a semi-structured interview. Personal thinking, including experiences, feelings, opinions, desires, and regrets was classified by objective methods, from which main concepts were extracted for the Value Chart. The feedback to the survey, including activities during the Covid-19 pandemic management, is classified according to the analytical methods of Kleinman, Frank, Bury and Launer-Robinson. Regarding sentiment analysis, the emotional and subjective context of the text provides an ideal baseline to validate the result. The analysis was implemented using neural networks trained on dictionaries and natural language (i.e., Tweets). The originality of the work lies in the fact that generally value charters are built on a Society's values. In contrast, in this case, individual contributions were gathered to complement the ethical values on which the society is founded. © 2022 IEEE.

ABSTRACT

Introduction: Legionella pneumophila is an opportunistic pathogen that is a key contributor to drinking water-associated disease outbreaks in the United States. Prolonged water stagnation periods in building plumbing systems due to low occupancy, especially during building shutdowns, breaks, and holidays, can lead to water quality deterioration and (re)colonization of buildings with L. pneumophila. Water monitoring in buildings typically relies on grab samples with small datasets. Methods: In this study, a larger dataset was created by sampling a Leadership in Energy and Environmental Design (LEED)-certified data-rich commercial building for L. pneumophila and physical-chemical water quality during the COVID-19 pandemic after reduced building occupancy. A proxy for human occupancy rates using WIFI logins was recorded throughout the study period. Results: L. pneumophila was observed in grab samples taken throughout the building, where concentrations generally increased with greater distances from the building point of entry to locations throughout the building. Factors conducive to microbial growth were identified in the building including fluctuations in water temperatures, lack of chlorine residual, a low water heater setpoint, colonized water-saving fixtures, prolonged stagnation throughout the building;especially in an expansion tank designed to reduce pressure issues during demand fluctuations, and the presence of oversized softener tanks with ion exchange resin that contributed to chlorine residual removal as well as colonization of the resin with L. pneumophila. Discussion: Flushing and thermal disinfection alone did not resolve the problem, and replacement of the expansion tank ultimately resolved the L. pneumophila issue. As ad-hoc approaches are logistically- and time-intensive, more proactive approaches are needed for informing preventative and corrective actions for reducing the risk of exposure to opportunistic pathogens in the building plumbing. ABSTRACT

Background-Aim: Cognitive impairment may represent a long lasting symptom after COVID-19 resolution and FDG brain PET is useful to evaluate if brain metabolic changes are transient or long lasting. Hypometabolism was shown in many brain areas, i.e. cingulate cortex, bilateral gyrus rectus, prefrontal and orbitofrontal cortex and cerebellar vermis. Methods: We report the case of a 62 years old man with type 2 diabetes, affected by COVID-19 infection in October 2020. After resolution, the patient had short-term memory loss and speech deficit affecting daily living and working activities and referred to the Gerontology and Geriatrics Institute (Univ. of Perugia). Neurological examination and neuropsychological tests were carried out and no alterations were found. In October 2021, the neurological examination was still normal, as well as neuropsychological tests. Brain MRI showed only two small chronic ischemic foci without bi-hemispheric white matter clinically significant abnormalities. In November 2021, the patient underwent FDG brain PET/CT (discovery ST, G.E.) according to standard protocols and images were evaluated both qualitatively and semiquantitatively. Results: An area of moderate significant hypometabolism was identified in the precuneus (predominant on the right side) and others multiple small and mild hypometabolic regions were localized in bilateral pre-frontal cortex, sensorimotor and parietal cortex both on left hemisphere. PET and MRI fusion images (Syngo.via VB10B image processing software, Siemens) showed that hypometabolic areas corresponded to structurally intact parenchyma at MRI. In January 2022 clinical and neuropsychological follow up did not evidence cognitive impairment, although the patient still felt depressed and impaired in memory, attention and daily living activities. Conclusions: In this case, FDG brain PET/CT was the only diagnostic procedure showing findings consistent with patient symptoms. In particular, precuneus hypometabolism may represent in this patient an early hallmark of dementia (i.e. Alzheimer's disease-AD), although other characteristic brain areas are not significantly impaired (i.e. cingulate cortex). In this case, FDG brain PET use, during follow up, could be crucial to evaluate if the metabolic changes may evolve into a chronic state, thus supporting mild cognitive impairment clinical suspect due to AD or confirming a stable COVID related neuronal damage. Furthermore, a second normal FDG brain PET/CT scan may suggest a post-acute infection transient phase, preluding to normal functional status. In conclusion, FDG brain PET/CT may represent an important diagnostic tool in modifying subsequent diagnostic assessment suggesting or routinely clinical follow up or other investigations for dementia (i.e. amyloid PET, amyloid and Tau protein liquor measurement). In our study, fused PET and MRI images were used, although hybrid PET/MRI system could be the choice option if available.
